You ask your employer to fill out a written accident report and you should absolutely ask your employer for a copy.  Quite frankly, there are some employers that will not do that, and if you’ve asked and they will not do it, there is obviously nothing you can do about that if your employer is being obstinate.  But you should definitely make the request, and if your employer will not fill out a written accident report and give you a copy, then you should fill out a statement of what happened yourself and give it to the employer and keep a copy yourself.  Prove you gave it to your employer by writing down the time and the date and the person you gave it to so there is no question that you reported the injury. You should do that as quickly as possible after the work injury.
